The forms below can be printed and filled out at home. Both forms are needed. They can either be turned in the day of registration (uniform sizing will be performed) or dropped off at the Recreation Department if child's uniform sizes are already known.

You may also drop off registration forms and payment to Carissa Oxford our Registration Coordinator. Please place your form and payment payable to Mimosa Booster Club (NO CASH) in a sealed envelope into the mailbox that states MBC at 72 St. Anthony Street.
